The Washington Township Health Department is pleased, once again, to offer the Adult Health Screening (SMAC).  This year, the clinic will be held on Saturday, November 9, 2019 between the hours of 8:00am and 10:00am at the Long Valley Rescue Squad building, 70 East Mill Rd. in Long Valley.

There are several bloodwork packages offered again this year:  Basic SMAC = $25.00;  SMAC + PSA = $40.00;  SMAC + T4 = $30.00;  SMAC + A1C = $40.00.

Pre-registration and payment is required prior to November 7, 2019.  Participants must come in to the Washington Township Health Department to pre-register and make payment.
